Welcome to Innovative Dentistry Biltmore! We’re excited for you to come in for your first appointment. You will receive New Patient intake forms with the appointment confirmation email we send. Below are the paper version of those forms. Be sure to complete the forms before your appointment so we can verify your insurance before your visit.
